HEART ATX Community Health Workers (CHWs) provide services in the community, Federally Qualified Health Centers, and affordable housing settings. Our CHWs promote COVID-19 vaccination and prevention, and address chronic disease risk factors and social determinants of health to build community resiliency. This program serves all of Travis County, with a focus on underserved communities and the 78702, 78719, 78721, 78723, 78724, 78725, 78741, 78742, 78744, 78752, 78753,78758, and 78617 zip codes.

Diabetes Prevention

Diabetes Management
Diabetes Empowerment Education Program (DEEP)
DEEP is a 6-series class on how to live well with type 2 diabetes, but is open to everyone. The series gives an overview of diabetes management and monitoring, exercise, nutrition, complications and prevention, and effective communication with health care providers. Register for DEEP.
